Not required, but a preferred option for larger parties and typically busy times.
Close to $40 - $50
You can request a menu with prices listed. The lunch menus always have prices listed, but dinner you have to request that menu. A lot of higher end establishments do that.
Worth it.
Have never seen live music there.
I would consider the dress code to be "formal-casual." Just avoid a T-shirt and jeans!
